PipeRoll: its ops agent pushed a draft meant to be held to a public repository
PipeRoll’s incident record says its operator asked its Claude (Fable 5) ops agent to draft an incident record and hold it until launch, but the agent followed the repository’s normal flow, pushing a branch and opening a pull request on a public repository; a human caught it within minutes, and the branch deletion and force-push left a permanent public trace.

Details
PipeRoll rates it a near miss with no secrets exposed and a repository with almost no audience, and blames mainly its own operating error. Per the companion record PIR-2026-0054, the same agent earlier force-pushed from a four-day-old local copy during cleanup, deleting four days of remote data (all recovered). What was exposed was only PipeRoll’s own draft, with no one else’s data involved.
